Player Contract*

I (Player), agree to play baseball/softball for the South Wake Storm Youth Sports Association for the up and coming season. By signing this contract, I will follow these expectations:

I will do my best to attend all practices, games and tournaments. I understand that playing time can and will be directly related to my attendance, performance, and attitude at practices, games, and tournaments and that playing time is NOT guaranteed.

Our family agrees to pay the non-refundable player fee as communicated from the Head Coach with this signed contract. I understand that this fee covers expenses for the remainder of the current season only, and that additional monies will be required for any future seasons. I understand that I may need to participate in fundraising activities throughout the season to help raise the needed funds to cover the team’s expenses. Note: uniforms will remain with player, but all other equipment and purchases made with team funds remain with the team and/or organization. Player will not be eligible to play in first game, or any thereafter, until payment in full is received. No exceptions.

I understand that all sponsorship/donation money raised by my family for my Storm team is for exclusive use by the Storm. It is non-refundable, and will no way reflect in my playing time.

I understand that by me and my parent/guardian signing this contract, I agree to not play baseball/ softball for any other recruited fastpitch softball or baseball organizations (with the exception of my school team) without permission from my coach.

I understand that it is expected that I will need to exhibit the highest level of sportsmanship while representing the South Wake Storm Youth Sports Association throughout my time with the team. Whenever the Storm is practicing, playing in a tournament, or resting at a hotel between games, it is expected that each player will be hard working, courteous, and an excellent example of how to play the game on the field, and how to represent the Storm Association off of it.

I understand that cheering and excitement is encouraged, but must always be displayed in a positive manner and never demeaning to any other players, coaches, or umpires. At games, only coaches, assistants, and players are allowed in dugouts and players shall remain in dugout, even when not playing, to cheer on their teammates. Issues with plays called and/or coaching decisions should never be brought up in front of players or other teammates and should be addressed outside of games AND after a 24-hour “cool off” period. Violation of any of the above may result in a suspension of my player.

I understand that the coaching staff of the South Wake Storm will periodically review the performance of all players on the team. Should the performance of a particular player be less than what is needed to contribute at the level of team play expected, the coaching staff reserves the right to remove said player from the roster. Refunds of any monies taken in by the team from the family would be returned on a prorated basis. In addition, if at any time I choose to leave the Storm, I am not entitled to a refund of monies paid to-date.

I understand the risk of injury from the activities involved in the program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment and personal discipline may reduce the risk, the risk of serious injury does exist. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, BOTH KNOWN AND UNKNOWN, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume all responsibility for any participation. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless the South Wake Storm Youth Sports Association, their officers, officials, agents, coaches, sponsors, and advertisers WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE. I understand that I must also notify the coaching staff of any injuries to my daughter/player that may affect her performance and/or jeopardize her long-term health and wellbeing. Coaching staff has full discretion on determining the player’s condition and ability to play or practice.

I hereby authorize the South Wake Storm Youth Sports Association, and team, to use, reproduce, and/ or publish photographs and/or video that may pertain to me or my daughter/player – including my image, likeness and/or voice without compensation. I understand that this material may be used in various publications, public news releases, recruitment materials, broadcast public videos, or for other related endeavors. This material may also appear on the South Wake Storm website, or other team-related social media sites. This authorization is continuous and may only be withdrawn by my specific, written rescission of this authorization. Consequently, the South Wake Storm, association and team, may publish materials, use my or my player’s name, photograph, and/or make reference to me in any manner that the South Wake Storm, association or team, deems appropriate in order to promote/ publicize the organization.

Upon signing this document, I agree to register my player with the organization on the www.southwakestorm.org website

By signing this player contract, I acknowledge that I have read in full the South Wake Storm Youth Sports Association By-laws and agree to abide by association policies as well as the provisions provided in this document.
